IMPORTANT NOTES: If you are going to be paying for more than one person, you must send in a SEPARATE FORM for each person that will attend the conference. After you download the form, please be sure that you READ ALL INSTRUCTIONS CAREFULLY. Be sure to mail the completed form(s) to the address specified on the form. Payment can be made by check or money order. If paying by personal check, please be sure that you have adequate funds to cover the check or we will be forced to pay a service fee for each bounced check. This will cause your registration to be lost and we will have to charge you the service fee if you want to re-register. Reminder: Hotel rooms are not provided as part of the registration fee.
