This is done with grace and beauty. The message is quite impressive. The only thing I noticed is you had the words moments in most of the verses but not all of them. For me this would have a far greater impact if you found a way to put moments in every stanza. This touched my heart and helped me to be more aware of God in the moments of my life. This would be beautiful on a thinking of you greeting card.
